Gwent 4YP

Platfform Gwent 4YP provides mental health and wellbeing support for young people living in Gwent. 

Gwent 4YP brings young people, aged 14 – 18, together to empower each other through the discovery of shared experiences and connection. We work with young people to develop skills and find ways that work for them to manage their wellbeing so that they can live healthier, happier, and more fulfilled lives. 

Our support gives young people a chance to share their experiences with others who may be facing similar challenges, and to learn new strategies to promote their wellbeing. They decide what areas of wellbeing they want to discuss and how much they want to share. 

It’s important for young people to know that whatever they’re facing, they’re not alone or isolated. We believe the power of peer support can be transformational and work to train young people who would like to become peer mentors.


What we offer

We offer a range of different support in a variety of settings the five local authority areas [Caerphilly, Newport, Torfaen, Monmouthshire and Blaenau Gwent] where young people can learn and share their experiences in a safe and relaxed setting. 

The following services ensure we have something for everyone, no matter what you’re facing. These include: 

  • One-to-one support 
  • Social & Community support 
  • School Workshops 

One-to-one support 

Our one-to-one support takes place over six, one-hour sessions. This session can be placed either face to face or online depending on where the young person feels most confident. Please note, this is not a counselling service. We focus on a strength-based approach to help the young people learn new ways to manage their mental health and wellbeing.  

Social & Community support 

We offer weekly and quarterly meetups facilitated by a member of the team. These are organised in a relaxed and welcoming environment where young people can feel safe, be their real self and have their own voice.  

School Workshops 

Alongside the community-based support we offer as described above. We also work in partnership with Secondary Schools across Gwent to deliver our State of Mind groups within school time. As part of this support, we can work with up to two groups of 15 young people over a six week period of time. If you’re interested in our support then please get in touch: lukerees@platfform.org

If you are a young person or a professional working with someone that could benefit from our support. Then you can use one of the following referrals routes to request support: 

For professionals 
Request for support from professionals for a young person can be made using this referral form. Referrals area also accepted through the SPACE Wellbeing Panel. 

For young people 
Young people can access the project directly by using this self-referral form

Wellbeing Practitioner - I work closely with YP to help them be the best versions of themselves and help them overcome any challenges/barriers that they may be facing
